The report of issuance of a policy and endorsement provided for in Sec. 703.116 or notice of cancellation provided for in Sec. 703.114 must be sent by the home office of the carrier, except that any carrier may authorize its agency or agencies in any compensation district to make such reports, provided the carrier notifies DLHWC of the agencies so duly authorized.
